We provide accessible referral pathways into care, supported by qualified clinicians and a dedicated team delivering ethical, connected support.

REFERRAL PATHWAYS

At Dynamic Connections Therapy, we offer flexible referral pathways depending on your needs, funding type, and the service you are seeking. You can refer yourself, a child, or a participant you are supporting.

If you’re unsure which pathway applies, you’re welcome to contact our team and we can guide you through it.

  • You are welcome to refer yourself, your child, or a family member directly to our service.

    Self-referrals are suitable for people seeking support with:

    • Emotional wellbeing and regulation

    • Developmental or behavioural concerns

    • Parenting support

    • Social and relationship difficulties

    • General therapy or counselling support

    Once we receive your referral, our team will:

    • Review your information

    • Contact you to discuss your needs and current availability

    • Arrange an intake appointment if appropriate

  • We accept referrals for participants under the NDIS, including children, teens, and adults.

    Referrals can be made by:

    • Support coordinators

    • Plan managers

    • Families and carers

    • Self-managed or plan-managed participants

    NDIS referrals are suitable for supports relating to goals such as:

    • Emotional regulation and mental health support

    • Social and communication skills

    • Daily living skills and independence

    • Sensory processing support

    • Capacity building and functional skill development

    After receiving your referral, we will:

    • Review the participant’s goals and supports

    • Confirm funding and service alignment

    • Contact you to discuss suitability and availability

  • Our Occupational Therapy services support children, teens, and adults to build confidence and independence in everyday life.

    OT referrals are suitable for support with:

    • Sensory processing and regulation

    • Emotional regulation strategies

    • Fine and gross motor skills

    • Daily routines and self-care skills

    • School participation and learning support

    • Attention, planning, and executive functioning

    You do not need a GP referral to access Occupational Therapy through private or NDIS funding.

    We accept referrals from:

    • Self-referrals (parents, caregivers, or adults)

    • NDIS participants (self-managed, plan-managed, or agency-managed with approval)

    • Support coordinators and allied health professionals

    Once received, we will:

    • Review your goals and needs

    • Contact you to discuss suitability and availability

    • Arrange an initial intake appointment if appropriate

  • For psychology services, a referral from a GP is required.

    A GP referral allows eligible clients to receive Medicare-subsidised psychology sessions.

    Your GP referral should include:

    • A Mental Health Treatment Plan (if applicable)

    • A referral to Dynamic Connections Therapy or your specific psychologist

    • Relevant background information to support your care

    If you are unsure how to obtain this, your GP can assist you in arranging it during an appointment.
